Accepted Insurances

Orthopedic & TMJ Physical Therapy Center accepts Providence insurance only; we are in-network for TMJ and out-of-network for other conditions. We are out-of-network with the Providence Intel and Connect (Neighborhood) plans regardless of diagnosis. We charge $170 for initial evaluations and $160 for follow-up visits. We can provide a superbill at each visit for submission of insurance reimbursement.
